APPOINTMENTS:
Appointments may be scheduled during our routine office hours.
PHONE CALLS:
If you wish to speak with a Medical Assistant or Dr. Moore, our receptionist will take your name and telephone number for your call to be returned. We try to return your call as quickly as possible and will make every effort
to return your call that day.
If you have a medical emergency, please go directly to the nearest
Emergency Room.
PRESCRIPTION REFILL REQUESTS:
Prescription refills are filled during regular office hours when Dr. Moore has access to your medical record. Please have the name and phone number of your pharmacy available at the time of your call.
OUR PAYMENT POLICY:
Any Co-Pay at the time of your appointment.
Any past due balance is due at the time of your appointment,
unless a payment plan has been arranged with Amy.
We accept MasterCard, Visa & American Express, Care Credit,
Cash or Debit Card.
Your cooperation is greatly appreciated.
OUR CANCELLATION POLICY:
If you are unable to keep an appointment, we ask that you
call our office 24 hours in advance.
This allows us to offer another patient who may need to be seen.

OUR APPOINTMENT NO SHOW POLICY:
A $ 40.00 No Show Fee may be charged for 1st No Show.
$ 50.00 No Show Fee for your 2nd No Show
If a 3rd No Show occurs, you may be dismissed from our practice.

As a courtesy, we confirm patient appointments 2 days before.
Please be sure we have your correct phone number on file.

We do not currently accept MDWise, HIP, Anthem HIP or Care Source.
If your insurance plan is not listed, please call our office and ask to
speak with Amy.
If your insurance requires a Referral to be seen by an OB/Gyn, please contact your Primary Care Provider and request a referral.
